The survey consists of 2 Parts. Part 1 asks general questions about your experience with leadership. Part 2 is optional, and asks specific questions about your church and church leadership, as well as your personal information, should you choose to give it. You are permitted to complete one survey only. Please do not try to complete multiple surveys.
Though it will not be published on the web, you are asked to give name the Calvary Leader to whom your problem pertains, as well as the name of the Calvary Chapel church. You are not required to give your name and email address. However, if you provide them, your name and email adresss will be held in strictest confidence. You will not be contacted by anyone from this website regarding your survey. Do not answer any questions on this survey based upon hearsay, but only if you are directly involved in a circumstance. Please complete all survey questions.
The purpose of this site is to gather information on leadership issues and trends with the hope that Calvary leadership will use it to make changes and correct error where it exists.
Who am I? My name is Scott Mahathey, I have created this website to help Calvary Chapel, not to defame it. Nobody at Calvary Chapel has enlisted me to undertake this study. I experienced a conflict with a Calvary Leader years ago and recently attempted to help others who underwent a time of hurting regarding a Calvary Leader. Many Calvary leaders desire to be upright and honest Christians of integrity. Our prayer is that this site will help them with that.
This is a survey site, not a blog. Some respondents may choose to air opinions about Calvary Chapel and its leadership publicly. If that is your desire, blogs are available for that. We recommend that you first approach the individual with the issue, then witnesses, then the Church, according to Matthew Chapter 18 prior to completing this questionnaire. Doing so is proper, and will enable a more complete survey response.
